Step 2. Hamilton James & Bruce’s Site Induction
There are several forms we request that you use to assist us in monitoring your health and safety in your place of work.
Site Induction Registration Form
This form should be completed on the first day of placement for every temporary or contracting assignment. You must complete this form on site with your supervisor, then fax the completed form to your HJB Bowdens consultant.

Hazard Identification Form
If you identify a hazard, it is your responsibility to notify us. Please complete the Hazard Identification Form to notify HJB Bowdens of any potential or existing hazards.
The form should be sent on to the OH&S Representative in the Brisbane office, Sharon Palmer, who will investigate the hazard and follow up with the client.
